Improving the Health of Vermonters

Our state has a long history of improving public health. Vermont was named the #1 healthiest state for the fourth year in a row in the 2012 America’s Health Rankings. But there are challenges ahead. With the publication of Healthy Vermonters 2020, we begin our third decade of assessing the health of Vermonters – and setting, measuring and working to achieve our shared goals for improving public health.

Healthy Vermonters 2020Healthy Vermonters 2020
is the State Health Assessment Plan that documents the health status of Vermonters at the start of the decade, and the population health indicators and goals that will guide the work of public health through 2020.

State Health Improvement PlanState Health Improvement Plan
is a five-year plan that prioritizes broad Healthy Vermonters 2020 goals: reducing prevalence of chronic disease, reducing prevalence of substance abuse and mental illness, and improving childhood immunizations. Includes strategies and planned interventions.

Health Department Strategic PlanHealth Department Strategic Plan
is a three-year plan that sets the strategic direction and priorities, based on Healthy Vermonters 2020 data, to guide the Health Department’s work, quality and system improvements.

The Health Department works to improve the health of Vermonters by regularly reporting on and applying data to make decisions.

Maps & TrendsHealth Department routinely uses three ways to assess population indicators (such as smoking prevalence) at the local level: by county, by district office area, and by hospital service area (HSA). Maps & Trends links you to maps, tables and graphs for all Healthy Vermonters 2020 indicators and goals.
