Online Licensing and Records

Members of the public can easily look up information about Vermont licensed health care professionals.

Health care professionals licensed or certified by the Board can apply for and renew a license or certification, check the status of an application, or update profile information.

Vermont Prescription Monitoring System (VPMS)
To access the system, simply click the "elicense" link above and log into your account.  There you can find the link to the VPMS registration.  If you experience any technical problems with the registration process, please email the VPMS Help Desk at or call the Help Desk at (888) 461-8628. If you are seeking to request a Verification of your Vermont medical license or certification, please submit your request in writing to our office (address below) indicating where you wish the verification to be mailed, and with a $50 payment (check, cash or omney order) payable to the Vermont Board of Medical Practice.

A Message from Commissioner Harry Chen, MD and the US Surgeon General's Call to End the Opioid Crisis

Vermont – like other states across the nation – has been struggling with an epidemic of opioid use disorder. This epidemic is caused in part by mis-prescribing or over-prescribing opioid drugs to treat pain.


All the evidence now shows that opioids can be addictive even when prescribed for legitimate pain, and even with the first dose taken. In Vermont, we are seeing lives damaged and lives lost – more than 400 deaths from opioid overdose since 2010.


We must stop this now. I ask you to join U.S. Surgeon General Vivek Murthy, MD, MBA and me in pledging your commitment to reverse this deadly trend. Please, take the pledge at Know how to treat pain safely and effectively. Screen every patient, every time. And treat opioid use disorder as the chronic disease that it is.
